Through the patient portal you can view your test results, access your medical records and send secure messages to our care team.

Once you provide us with your email address, you will receive an email with your username and a link to set up your patient portal account. If you did not receive it, please call us at 773-394-2888. You will then verify your identity with a code sent to your phone and create your own password.

Click "Forgot password" or "Trouble logging in?" on the portal login page to reset it online. If you still cannot log in, call our office and we can help reset your account.

Most lab results are available in your patient portal within 7–10 business days. If your results need discussion or a follow-up appointment, we will contact you by phone or through the portal to schedule an appointment.
