What's included

What the exam covers

At Lau Medical, we follow the CDC's Technical Instructions for Civil Surgeons to provide a thorough, personalized immigration medical examination. From reviewing your medical records to completing Form I-693, our team guides you through each step of the process.

Medical History & Vaccination Record Review

Before your visit, we review your history and vaccine records against the CDC's age-appropriate requirements to identify what may still be needed.

Physical Examination

Our USCIS-designated civil surgeon assesses your heart, lungs, eyes, ears, skin and lymph nodes, with mental health and substance use screening as required by the CDC.

Tuberculosis (TB) Screening

An IGRA blood test is required for applicants age 2 and older, subject to limited CDC exceptions. A positive result may require a chest X-ray and further evaluation.

Syphilis Screening

Blood testing is performed for the age groups required by the CDC, or when otherwise medically indicated. Testing is available through our on-site lab.

Gonorrhea Screening

Urine screening is performed for the age groups required by the CDC, or when otherwise medically indicated. A sample can be collected at our clinic.

Personalized Vaccination Recommendations

We discuss whether an antibody titer can demonstrate immunity or vaccination is needed. A seasonal flu vaccine may be required. We administer most required vaccines on-site.

Questions

Immigration Exam FAQs

What is a USCIS civil surgeon?

A civil surgeon is a licensed physician designated by U.S. Citizenship and Immigration Services (USCIS) to perform the immigration medical exam and complete Form I-693. Only a designated civil surgeon can sign the form β€” a regular doctor's physical will not be accepted.

Who needs an immigration medical exam?

Most people applying for a green card from inside the United States (adjustment of status, Form I-485) must submit a completed Form I-693. Some other immigration applications also require it. If you are unsure, check your USCIS instructions or ask your immigration attorney.

What tests are included?

The exam includes a medical history review, physical exam, TB blood test for applicants age 2 and older, and syphilis and gonorrhea testing when required by age or medical indication. Your vaccination record is also reviewed.

What vaccines are required?

You must show that you have received the vaccines that are age-appropriate for you under CDC requirements. If a dose is missing, you only need to receive one dose of that vaccine at your visit β€” you do not need to finish the full series before the form can be signed. We offer most vaccines on-site.

How many visits does it take?

You will need at least two visits: the first for the exam, bloodwork and any vaccines, and a second to pick up the sealed Form I-693. Additional follow-up may be needed if a test result requires further evaluation, such as a chest X-ray.

Does insurance cover the immigration medical exam?

The immigration exam is usually not covered by health insurance because it is required for an application rather than for medical care. Please call us at 773-394-2888 for current pricing. Vaccines or titers may be billed to in-network insurance.
